Almost Home Kids Provides:

  • A Care Coordination Program for Families whose children have special health care needs.
  • Medical Resource Libraries and Care Coordination Services for physician's offices.
  • "CoACH Approach" Training programs for families and health care providers.
  • A Midwest Conference for Children with Special Health Care Needs.
  • Direct patient care services at our children's Center, in Naperville, Illinois.
  • Consulting Services to support the Center development nationwide.
  • Speaking from the Heart - guest speakers for events, conferences, and more.

Respite Care

The Respite Care Program welcomes children for time periods from overnight to 14 days each year. The child will be provided with highly skilled clinical care and recreational activities in a secure and therapeutic environment, while their parents/caregivers tend to personal or family issues. Families may choose Almost Home Kids for respite care for variety of circumstances: Open nursing shifts, parent illness or need for surgery, vacations, family emergencies and much needed sleep/rest.


Transition Care and Training

This program transitions medically fragile children from the hospital intensive care units to home, using a multidisciplinary team approach to train the parents on the care of their child. Children at the Center receive physical, occupational, speech and medical massage therapy to improve functional skills. The programs also works to ensure that when the child is ready to go home, all needed services and accommodations are in place. These include home health nursing, physical/occupational/speech therapy, electrical sysem upgrades and physical adaptations to the home.

Medical Home Care Organizer

Introduction

This Organizer was planned with family-centered and family-sensitive care in mind. We believe that if empowered and given the correct tools, parents will become stronger partners with their child’s primary care physician and other health care professionals and educators. This teamwork ensures a seamless quality care coordination process that meets the needs of the family and child. You will see that the Organizer combines health, education, community activities, therapies and family to ensure that you can meet the needs of the "total child".

Using the tabs on the top of this page, consider the section or sections that apply to your child. If your child requires extensive medical management and has home care services that include private duty nursing and medical supplies for example, the contents of the entire Organizer can be useful. If your child has less intense needs but nonetheless requires medical management in the areas of medications, therapies or education goals, then choosing certain sections of the Organizer may be easier to manage.

For Parents / Caregivers

We hope you enjoy using this Care Organizer. Using it ensures that the child's care can be managed in a way that provides quality care for the "whole child". By implementing this tool on a continued basis the organizer can prevent duplicated services, eliminate fragmentation and offer critical information during emergencies that could save a child's life.

Often times the stresses of caring for a child with special health care needs (CSHCN) stems from not remembering what needs to be done, forgetting to follow up on an appointment, worrying about a potential crisis and not being prepared to respond with accurate information. To eliminate these stressors means a better quality of life for the entire family.

Although this organizer can be tedious to fill out initially, the goal is that once it's up to date and can be managed over time, it will make life easier for the family.

The Medical Home concept strives to serve and promote a better quality of life for children and youth with special health care needs and enhances the family's relationship and communication with their primary care physician and other specialists/therapists caring for their child.

The Care Organizer is simple to navigate. The form titles are listed in categories that are easy to understand and access. Each form has data that provides you with a clear picture of the total child while making care giving and medical management easier and safer to provide.
